Procedure for Requesting Transcript: an attorney or self-represented party may use this form to request a transcript of his/her proceeding. The form may be submitted on-line, by completing the fields below, and clicking "submit" at the bottom of the page.

If requesting transcripts for multiple hearings in one case, you may submit one form with clearly marked hearing dates. If requesting transcripts for multiple cases, you may use one form with clearly marked case numbers and dates.
Fees: Upon submission of the Request, you will be contacted with an estimated transcript cost, associated fees, and the deposit amount.
Transcript Completion: Time limits for delivery of transcripts are statutory. The deadline is calculated from the date of receipt of the deposit. Once the transcript is completed and all fees have been paid, the requesting party will be notified when the requested transcripts have been filed. Copies of filed transcripts will be delivered to the requesting party in accordance with the method of delivery stated on the Transcript Request Form.
